02:53:56am		I was wondering if there is any information on the quantity of this envelope printed?

Also, any information on what the 2.1c rate covered (e.g. had to be in a batch of 100, or just single usage OK)?

According to the 2016 Scott Specialized Catalog of US Stamps and Covers,

there were 120,280 of these envelopes printed.

The 120,280 refers to the number of first day cancellations applied to U578 in Houston, Texas.
