I saw this cover at our auction last night and I guess I was the only one curious. I don't know if this cover was contrived or if people did this sort of thing ?

Roy is correct, the dispensing machines often screwed up, many used 'grippers' to advance the coil stamp to the cutter. I have the example below which looks to be a 'bi-sect' usage but really is not, it is simply a misfeed from the dispensing machine.
If you look closely, you can see the grippers have actually damaged the stamp when it tried to pull the coil roll forward. I think this happened when the coil roll got close to the end of the roll.
