It has thus far been my experience to find that CTOs are dated to match the year of issue.
I have one stamp with the "cancellation" in the position generally found on a CTO but in a different year. One was issued in May 1969 but the cancellation is 1970.
Any thoughts? Could it have been postally used?

The eastern European countries I think cto'd stamps in response to demand. When they were all sold out they just postmarked another batch of sheets, and of course often postmarked remainders for cto use after the issue was replaced.

I'd say it can vary a lot depending on the country, period of issue, printer etc.
For example a good number of Soviet stamps of the 1960/70s were CTOd back in the year of issue, but quite many also exist CTO'd the following year or two, but also up the 1980s and even 1990s though in much smaller quantities. And Romania had somewhat similar CTO practices.
Some more oddball experiences have been with some French African (Togo, CAR etc) stamps with CTO-dates before the official day of release. Talk about stuff
